Roaches fall out of paper towel dispenser in Miami-Dade; rodent issues found inside Key Largo restaurant

Below is a list of places that were ordered shut in South Florida last week.

Rodent activity was spotted inside Sushi Sake in Key Largo.

All the places mentioned were allowed to re-open following an ordered cleanup and re-inspection.

“Roach activity present as evidenced by live roaches found. Observed more than 3 live roaches inside the paper towels dispenser. When I pulled the paper towels , 3 roaches fell from the dispenser. Also observed 1 live roach crawling on the floor of the kitchen and 5 live roaches inside the reach in cooler that is not working located in front of the stove.”

“Nonfood-grade bags used in direct contact with food. Observed food stored on thank you bags at walk in freezer and walk in cooler.”

“Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed pork chops (124°F - Hot Holding)in the steam table at the front counter. As per employee less than 2 hours. Employee was told to reheat the food.”

“Manager or person in charge lacking proof of food manager certification. Repeat Violation.”

“Operating with an expired Division of Hotels and Restaurants license. Observed license expired 10/2023.”

“Rodent activity present as evidenced by rodent droppings found. Observed approximately 13 rodent droppings on top of the dishwasher machine, also observed approximately 9 rodent droppings on the floor under dishwasher machine.”

“Buildup of food debris/soil residue on equipment door handles. Observed in unit at sushi station.”

“Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed at the preparation area.”

“Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed cooked tempura shrimp (52F - Cold Holding) in reach in cooler at the sushi station, as per employee over 24 hours. Employee discarded during inspection.”

“Time/temperature control for safety food thawed in an improper manner. Observed raw shrimp (frozen) bags in a container inside hand wash sink at the back of the kitchen.”

“No currently certified food service manager on duty with four or more employees engaged in food preparation/handling.”

Observed one live roach crawling on the wall underneath the three compartment sink located in the kitchen, observed 1 live roach crawling on an empty refrigerated drawer underneath the burner located in the kitchen, observed 1 live roach crawling on the kitchen floor in the middle of the kitchen, observed 3 live roaches crawling on the floor in between the two reach in coolers in the kitchen, observed 5 live roaches crawling on the wall and floor under the reach in cooler by the service window, observed 1 live roach crawling on the hinge of the reach in cooler door by the service window, not touching food.”

“Dead roaches on premises. During the inspection observed 17 dead roaches on the floor and also floating on a plastic container filled with water behind the upright reach in cooler located in the kitchen, observed 7 dead roaches on the floor underneath the three compartment sink located in the kitchen, observed 2 dead roaches crushed against the gasket on an empty refrigerated drawer located underneath the burner in the kitchen.”

“Floor soiled/has accumulation of debris. Observed heavy grease and old food accumulation underneath the oven and underneath all cooking equipment in the kitchen.”

“Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed heavy soiled and grease accumulation on the storage shelves above the three compartment sink, observed heavy grease accumulation on a small storage shelves located in front of the steam table in the kitchen.”

“Operating with an expired Division of Hotels and Restaurants license.”

“Hand wash sink used for purposes other than hand washing. Observed cutting boards drying inside hand wash sink at front counter.”

“No soap provided at hand wash sink. Hand wash sink located in the kitchen.”

“Observed 1 live roach crawling on the wall behind the conventional oven located at kitchen, observed approximately 20+ live roaches crawling between the wall and a PVC pipe on the ceiling all around the kitchen.”

At the steam table cutting board, 1 live roach At the floor next to the pasta station flip top cooler, 1 live roach **Warning**”

“Clams/mussels/oysters tag removed from original container prior to container being emptied. At cook line, clams and mussels no original tags until last one used. Cook put tags back into containers in flip top cooler.”
